If your dental benefits include assignment of benefits, we are happy to bill your insurance provider directly on your behalf. To help ensure a smooth check-in process, please have your insurance policy number, benefit plan information, and, if applicable, your student ID available when you arrive for your appointment.
Please note that any portion of your treatment not covered by your insurance plan, including deductibles, co-payments, coinsurance, or fees that exceed your coverage, is the patient's responsibility and is due at the time of your appointment.

If your dental benefits do not include assignment of benefits, payment in full is required at the time of your appointment. You will then submit your claim to your insurance provider for reimbursement.

We are pleased to accept patients covered under the Ontario Disability Support Program (ODSP) and Healthy Smiles Ontario.
Please bring your valid benefits card and ensure your coverage information is current for the month of your appointment. If your benefits cannot be verified or your coverage is inactive, payment for your treatment will be required at the time of your appointment. Patients are also required to sign the applicable claim forms for services provided.

We are pleased to accept patients covered under the Canadian Dental Care Plan (CDCP). Please bring your valid CDCP information to your appointment so we can verify your coverage.
Please note that the CDCP may not cover the full cost of all dental services. Any treatment not covered by the plan, including co-payments or services that exceed your coverage, is the patient's responsibility and is due at the time of your appointment.
